Within the western world, America is one of the highest per capita healthcare spending. Unfortunately, increased spending has not correlated with better outcomes. In the area of chronic conditions, the troubling trend of rising costs and poor outcomes is noticeable. Followed by stroke, hypertension, and heart disease numbers in diabetes prevalence are steadily climbing. Therefore, America’s costly chronic disease problem has forced us to reevaluate the way we pay for care, measure quality, and manage chronic conditions.
