Meeting the Requirements of the Oncology Care Model (OCM)

04.17.17

Studies show that cancer patients’ access and adherence to treatment is often limited by physical, logistical, and psychological barriers. In 2012, the Commission on Cancer (CoC) moved to address these barriers with Standards for Continuum of Care Services, requiring clinics to:
(3.1) have a patient
navigation process to
address health care
disparities and barriers to
care, and
(3.2) screen for patients
with psycho-social distress.2
Additionally, in 2015, Medicare
came out with the Oncology
Care Model (OCM), which
established quality measures
with regards to:
? documented plan of care
for pain,
? patient experience and
satisfaction with care,
? assessment using
patient-reported
outcomes tools,
? and psycho-social
screening and
intervention.

Pack Health offers a simple way for clinics to extend their care beyond clinic doors and measure up to OCM standards. To learn more.
